Liaison Program:

As part of our recovery initiative, “Road to Recovery”, we have launched the exciting Liaison Project throughout New Passages’ Community Housing Services to facilitate the greater engagement of relatives and significant others. The Liaison Program was designed to establish this greater rapport with natural supports by building a solid and consistent communication network.  One feature of this program is having each facility send out a monthly newsletter to the consumer’s natural supports (guardians, family and friends) highlighting the program’s happenings.  

Case Management  & ACTP  Services:

Case Management continues to be a key element in the continued growth of clinical services within New Passages.  The Adult Case Management Program, in Genesee County, expanded by 30% during the last year.  Quality scores from both internal and external reviews showed significant improvement, with consumer satisfaction scores remaining high. In 2006, Case Management provided services to approximately 1,340 consumers. The New Passages’ ACTP, in Genesee County, was recognized for continued quality improvement, through its’ leadership in fidelity scores compiled via external audits of the program , and provided services to approximately 120 consumers in 2006.  The Team has been actively engaging in training to enhance their ability to provide integrated services for persons with dual disorders.  ACTP also recently moved into offices at the Carriage Factory that will put them in closer proximity to the Mobile Crisis Intervention and Stabilization Program, bringing together more resources to help reduce unnecessary psychiatric hospitalizations through an array of alternative services.  Mobile Crisis & Crisis Stab together provided services to approximately 5,994 persons in crisis in 2006. 

Community Housing:  

New Passages continues to provide specialized residential services in clean, safe, and homey environments, serving approximately 250 consumers in 2007. These licensed, adult foster-care homes are located in:  Clinton, Eaton, Ingham, Bay, Saginaw, Genesee, Sanilac, Oakland, and Macomb Counties.  A psychosocial rehabilitation approach is used in programs serving persons with mental illness to encourage the recovery process, while a similar approach is used in programs serving persons with developmental disabilities to help maximize potential and community integration.   

New Passages Outpatient Clinics: 

Adult Outpatient Services (AOPS) 

New Passages operates three AOPS clinics, two in Macomb County and one in Genesee County.  The clinics in Macomb County were renovated to create new office space for the addition of new staff and expansion of services.  The Genesee County AOPS clinic was moved to the Carriage Factory to put it in closer proximity to the array of adult mental health services offered by New Passages, and served approximately 1,373 consumers in 2006.  All three clinics began providing services for persons referred through the Michigan Prisoner Re-Entry Initiative (MPRI) and the Genesee clinic was recently awarded the contract to provide services for the Adult Basic Waiver (ABW) clientele.  

Children’s Services: 

New Passages provides mental health services for children and family through two clinics in Macomb County and one clinic in Genesee County.  Each of these clinics features the services of a Board Certified Child Psychiatrist.  New Passages also launched its plan to create a free-standing, Children’s Mental Health Center in Genesee County, at our Corunna Road facility.  This facility provided services to approximately 49 children in 2006.   Services currently include: psychiatric consultation and medication management, individual and family therapy, and case management services.  New Passages is committed to expanding and improving mental health programs to address this underserved population. 

Expanded Outpatient Services:

New Passages participated in the planning and development of several new case management programs in Macomb County, to be initiated in 2007.  Programs include: an IDDT-Intensive Case Management Team (providing intensive outreach services for sixty to eighty persons with co-occurring disorders), a Mobile Homeless Outreach Team (bringing services into the community to identify and assist homeless persons struggling with mental illness, in partnership with Macomb County Community Mental Health Services and member organizations of the Macomb County Homeless Coalition), and expanded Targeted Case Management (to provide more case management service capacity for persons with severe, persistent mental illness) at both of our Macomb County Outpatient Clinics. 

Psychosocial  Rehabilitation & Non-Vocational Services: New Passages manages two clubhouses: Friendship House, in Macomb County, and J-Town, in Jackson/Hillsdale County.  Membership and daily attendance in each of these vibrant clubhouses has continued to surge.  The J-Town Clubhouse was recognized for excellence by LifeWays for the three consecutive years and continues to rapidly grow as word of their positive outcomes expand to consumers and other service providers in the area.  The Friendship House continues to be the stable backbone of the psychosocial rehabilitation services offered by New Passages in Macomb County, boasting a membership in excess of 100 members. 

 

Portable Supports Services:

New Passages provides community supported living programs in two counties: Macomb and Livingston.  The Macomb County Portable Supports Program was nationally recognized for excellence and innovation.  It provides scattered site apartment living for around 30 persons who were able to move from Adult Foster Care programs.  The unique combination of assertive case management, direct care supports, transportation assistance, rental subsidy, and psychiatric medication management have made this a highly effective and successful program.  The Livingston County Community Supported Living Program continued to grow, moving to an expanded office to provide non-vocational activities and continuing to provide direct supports for numerous persons in apartments throughout the county. 

Crisis Services:

New Passages manages three CRU’s, two in Genesee County, and one in Saginaw County.  This provides New Passages with a unique regional perspective on the need for crisis beds.  In addition to the counties listed above, many of the surrounding rural counties (Lapeer, Sanilac, Tuscola, Shiawassee, and Livingston) have either inquired about or utilized these services on a contractual basis.  Utilization of these crisis beds has increased by about 25% over the last two years.
